FollowMD works to support stroke clinics and family doctors in providing frequent follow up care to individuals at high vascular risk.

Please fax a referral (see form below), along with a recent note or discharge summary, for secondary prevention — toll-free, to: 1-855-395-0883.

PLEASE NOTE: We do not provide urgent assessments of new symptoms – this must be done through an emergency room (ACT-FAST, call 911 for new stroke symptoms). New non-urgent (i.e. non-stroke) symptoms should be assessed through primary care. If in-person diagnostic tests are needed (e.g. MRI, echocardiogram), they must be arranged through primary care or existing high-risk TIA or stroke prevention clinics associated with regional or district stroke centres across Ontario.
