What is FollowMD?


FollowMD is a virtual specialty clinic, providing innovative and accessible long-term follow-up to reduce people’s risk of stroke and heart disease. 


We work collaboratively with both the existing stroke system and patients’ primary care providers to support ongoing focused follow-up care for patients who have suffered a stroke or transient ischemic attack (TIA). We are not primary care providers — our specialist physicians work closely with each patient’s primary care team. For those without a primary care physician, we focus on stroke risk factors, while supporting patients to access primary care in their community.

What is the cost?


We provide OHIP-funded medical care, which means that Ontario residents do not have to pay out of pocket for their doctor’s appointment. No Ontario resident will be turned away for inability to pay for physician services.  Non-OHIP patients can be seen via fee-for-service. Additional supports, including online educational programs, physiotherapist visits or dietician visits, are available via fee-for-service.

Who would benefit from FollowMD?


Individuals who have suffered a high risk event (stroke or TIA, peripheral arterial disease, hypertensive disorders of pregnancy) and would like support to reduce their risk factors for having another high-risk event would benefit from FollowMD’s frequent and focused follow-up appointments. 


After your first appointment reviewing your health history and risk factors, each future appointment will focus on one key factor at at time. This focus on one key factor at a time, combined with frequent follow-ups, helps support our patients to stay on track and achieve their goals.

How do I communicate with my doctor?


Patient-doctor appointments and communications are through a secure web-based or app-based videoconferencing platform that can be accessed on a smartphone, tablet or computer. The office staff can be reached by toll-free phone calls, or e-mail.
